FINANCIAL ELIGIBILITY REQUIREMENTS

Financial eligibility is determined by the verification of the applicant’s Medicaid status by the Texas Health and Human Services.

OTHER ELIGIBILITY REQUIREMENTS

• Be age 21 or older or be enrolled in the Consumer Attendant Services program.
• Need help with at least one personal care task
• Have an unmet need for medical services
